The purpose of this Survey is to help NCCPAP determine if it should seek to become qualified to administer peer reviews in New York State: (Please Note: ALL QUESTIONS MUST BE ANSWERED FOR SURVEY INFORMATION TO BE PROCESSED)

1. Is your firm an AICPA member? Yes (if YES, pls answer questions 2,3 and 4 with N/A) No

2. If your firm is not an AICPA member, has your firm been peer reviewed in the last three years? Yes (if YES, pls answer questions 3 and 4 with N/A) No Not Applicable

3. If your firm has not been Peer Reviewed within the last three years, does it plan to be peer reviewed in 2010 or 2011? (Note that your firm may be required to be peer reviewed by NYS legislation which mandates that CPA firms with more than two CPAs must be peer reviewed by 2010. Also, you may elect to be peer reviewed as an alternative to meeting the CPA competency regulation. It requires individual CPAs who perform attest services to have completed at least 1,000 hours of attest work in Yes (if YES, pls answer question 4 with N/A) No Not Applicable
